Open Access Letter
This letter is useful in children with heart defects where there is a possibility that they may require a more timely review at their local hospital. The local paediatrician with expertise in cardiology (PEC) will be responsible for agreeing this process for named patients only. Please be aware that not all Level 3 services can support ‘open access’ and urgent review may have to be sought via the GP or emergency services if appropriate.
Referral Form for Urgent Advice
This form is to be used when urgent advice from our on-call teams is required. This form should also be used for patients requiring transfer to Alder Hey Children’s Hospital ( Level 1) or Royal Manchester Children’s Hospital (Level 2).

Level 2
Send to mft.rmchcard@nhs.net before bleeping the on-call registrar via the hospital switch board 0161 276 1234
