Clinical Urgency Update Form

Please use this form to alert the team at Alder Hey Children’s Hospital that a baby or child who is on the waiting list for surgery or intervention needs to be reviewed because their clinical condition has changed. Please email this form to ahc-tr.cardiology@nhs.net

Open Access Letter

This letter is useful in children with heart defects where there is a possibility that they may require a more timely review at their local hospital. The local paediatrician with expertise in cardiology (PEC) will be responsible for agreeing this process for named patients only. Please be aware that not all Level 3 services can support ‘open access’ and urgent review may have to be sought via the GP or emergency services if appropriate.
