Supporting Your Journey to Adult Heart Services
The Hearts4Teens programme supports young people transitioning from paediatric to adult heart services by providing guidance, education, and emotional support. Starting from age 13, young people are invited to attend specialised clinics where they can learn about their heart condition, medications, and how to maintain a healthy lifestyle. The programme ensures a smooth handover to adult care, with dedicated specialist nurses assisting throughout the process. By empowering young people to take control of their health, Hearts4Teens helps them build confidence and prepare for adulthood with the necessary tools and support for ongoing care.
